Low Vision Clinic of Noor Subspecialty Eye Hospital is the first specialized low vision clinic established in Iran. The center is proud to bring together, along with the use of experienced experts in low vision, all the facilities and equipment needed to examine and modify the lifestyle of people with low vision, and hopes to provide better living and work conditions for these patients.

 

Low vision is also known as visual impairment, subnormal vision, and partial sight. Visual acuity is the most significant parameter that is measured in patients with low vision. The presence or absence of visual impairment is determined by the degree of visual acuity so that generally visual acuity less than 20/40 to 20/70 (6/12 to 6/21) is considered as low vision. According to the World Health Organization visual impairment has been defined as a best corrected visual acuity of less than 20/60 but not less than 20/400 in the better eye. Some references consider other factors such as contrast, reduced color recognition ability, disturbances in eye movement, glare sensitivity, and binocular vision reduction as important indicators of low vision. Visual impairment disorders can be examined in terms of aspects of medical, medical engineering, optics, social, economic, and even cultural. To help low vision people, the needs and performance of these patients in the home, workplace, public spaces and entertainment places should have been carefully considered, patient's difficulties and limitations should have been properly identified, prioritized, and then make a plan for these patients to use the visual aids based on this information.

 

Noor Eye Hospital has been honored to serve "Home Visiting" services since April, 2014. These services are provided especially for elderly low vision people who are weak physically and have problems to attend in the clinic.

 

The most important goal of studying the history and previous examinations of low vision people is to become familiar with the needs, limitations, and the patients’ visual potential. Some important factors such as how to behavior, how to move from the waiting room to the examination room, how to sit on the chair, the ability to find the path and identify the objects on the way, head movement, head and eyes position and, in general, all the sensory and motor symbols of the patients, are considered as important tips to determining the procedure for examining and prescribing the appropriate low-vision aids.

Measuring visual acuity with Snellen charts and logarithms to calculate required magnification for low vision people is the first step in prescribing low vision aids. Objective determination of refraction, and especially using radical retinoscopy, is a common objective method of determining the refractive errors in patients with low vision. For low vision people, subjective refraction is far more important than retinoscopy, so subjective tests for patients with low vision are the prerequisite for starting their clinical examinations. Additional tests such as color vision tests, contrast sensitivity, biomicroscopy and fundoscopy, and most importantly, visual field testing are essential tests for low vision people.

 

Before prescribing low-vision aids, it should be noted that the purpose of prescribing these devices is to help improve the patients’ performance for particular important activities, such as seeing distant driving symptoms or improving the patients’ writing ability. The patients should not expect that all their vision problems can be resolved only by using theses low-vision aids. For this purpose, the ophthalmologist should inform the patient about the limitations of these low vision aids and reduce the patients ‘expectations. The most commonly used method of prescribing low vision aids is to prescribe magnification devices. However, in terms of optics, the magnification is classified into three transverse, longitudinal and angular types, but for prescribing low vision aids, this classification is as follows:

 

• Relative distance magnification (RDM)

• Relative magnification (RSM)

• Angular magnification (AM)

• Electronic magnification (EM)

 

Optical equipment used for nearsightedness includes microscopes, magnifying glasses, base magnifiers, tele-microscopes and binocular loops. Choosing the proper type of visual aid device for nearsightedness is based on the magnification and type of patient's eye activity.

 

Telescope

The most important visual aids for far distances are telescopes. Telescope can be prescribed either as monocular or binoculars, hand-held or in the form of glasses. The optics system of telescopes is of two types: Galilean telescope and Keplerian telescope.

With the same magnification, Galilean telescopes are lighter, smaller, and cheaper than Keplerian telescopes, but the most important advantage of Keplerian telescopes is their wider field of vision compared to Galilean telescopes. In all types of telescopes, there is an opposite relationship between their magnification and field of view. The greater the magnification, the smaller the field of view; so the used telescopes do not usually have a magnification of more than 10X.

The telescope can be mounted in the primary position in glasses or the perimeter of glasses for occasional vision.

 

Color and Achromatic Filters

The use of achromatic and color filters is one of the ways to improve the function of patients with low vision in order to reduce glare and improve contrast sensitivity. These filters are used with different densities and sizes, alone or in combination with other visual aids. The color filters applied are based on the color vision deficiency. Because in low vision people, color vision deficiency causes affected individuals have trouble following the classic patterns of green-red or yellow-blue, the color filters used should be selected and prescribed experimentally.

Electronic Visual Aids

Optical systems do not often produce magnification larger than 14X. But patients with low vision may need more magnification. On the other hand, in conventional optical systems, the field of view, working distance, magnification, contrast and color are not much adjustable or altered. These limitations seriously raise the need for the use of electronic visual aids. The new electronic systems are relatively inexpensive, with features such as multiple color modes, removing software undesirable lines, auto- focusing mechanism, dynamic brightness and contrast adjustment, the transformation of the negative image into positive one ( and vice versa) and with many other image processing capabilities can provide better images for low vision people.

 

Life- Aid Supplies

In addition to the facilities described above, the equipment and devices that people with low vision use in their everyday life, such as telephones, television remote controls, nail clippers and etc. should be designed and made in accordance with their visual conditions. In standard low-vision clinics, in addition to providing visual aids, life- aid supplies should also be provided to the patients. These devices can improve the living conditions and independence of these people.

 

Rehabilitation Services

In addition to visual aids for independence of low vision people, providing rehabilitation services is an essential component of the low vision clinic. Behavioral, sensory and motor training, and rehabilitation of low vision people with different cognitive methods can help improve the efficacy of the treatment and the patients' independence. Generally the success of a low vision clinic depends on following items:

1-Skill, knowledge and experience of an optometrist or any other examiner working in a low vision clinic

2- Appropriate up-to-date facilities and equipment for low vision people

3-Patient motivation and attitude

4-Patients vision conditions and their previous experience in using visual aids

 

In aspheric glasses, in the range of +4 to +10 and brown and crystalline colors, in order to control convergence in close distance, 8BI to 12BI prism lenses have been pre-embedded.

High-power desktop magnifiers have two aspheric surfaces that reduce color errors at the edge of the lens and reduce distortion and increase visibility. Because these magnifiers are placed on the text, they are useful for the people who have problem of shake hand and are not able to hold a magnifier in their hands.
