Endophthalmitis treated by vitrectomy associated with fewer reoperations

Researchers reviewed medical records of 88 patients (88 eyes) with acute endophthalmitis after cataract surgery. Those with light perception only underwent pars plana vitrectomy (PPV), while those with hand motions or better visual acuity underwent either intraocular antibiotic injection or vitrectomy. Six of 37 (16.2 percent) vitrectomy patients required one or more additional procedures within 60 hours, while 28 of 51 eyes (54.9 percent) in the antibiotic group underwent PPV within 60 hours. The rate of response to primary PPV was significantly higher than to primary antibiotics (P <0.001).

Posterior sub-tenon Triamcinolone before panretinal photocoagulation may protect against visual loss, macular thickening

Researchers randomized 41 patients with bilateral severe nonproliferative diabetic retinopathy or proliferative diabetic retinopathy to a single posterior sub-Tenon capsule injection of triamcinolone acetonide in one eye and no injection in the contralateral eye before panretinal photocoagulation. From baseline to six months, there was statistically significantly less change in logMAR BCVA and foveal thickness in the treatment group than in the control group. The authors conclude that the triamcinolone prevents PRP-induced macular thickening, and thereby results in less visual loss.

Low birth weight may adversely influence optic nerve head parameters

In a large cohort of 12-year-old children, researchers examined the relationship between markers of poorer intrauterine growth – birth weight, birth length, and head circumference – and current optic disc parameters as measured by OCT. Birth weight was significantly positively associated with vertical optic disc diameter and inversely associated with vertical optic cup diameter and vertical cup-to-disc ratio. Smaller birth length and head circumference were significantly associated with larger cup-to-disc ratio. The authors conclude that fetal growth restriction may affect future glaucomatous optic neuropathy risk.

Sleep apnea may increase risk of proliferative diabetic retinopathy

Researchers used a standard screening device for sleep-disordered breathing to evaluate 166 surgical inpatients, 48 with nonproliferative diabetic retinopathy (NPDR) and 118 patients with proliferative diabetic retinopathy (PDR). Twenty-nine percent of the NPDR and 48 percent of the PDR patients were diagnosed as having the disorder. The authors conclude that the repetitive desaturation and reoxygenation associated with this condition may accelerate the progression of DR.
